Lyme Disease Symptom Checklist

[Librarian's Note: This is a simple checklist of symptoms to help a person gague how many varied symptoms may be related to Lyme Disease. There are checkboxes, and one should check all that apply. Once completed, one can bring this completed checklist to a Lyme-literate doctor, as this may alleviate needless and frustrating trips to medical offices wherein numerous peripheral diagnoses are made, but the underlying cause (Lyme disease itself) is missed.

If one thinks that one may have Lyme disease, it is imperative to have a blood test, namely either the ELISA or Western Blot tests to find out for sure.]
